There are three methods to submit non-electronic forms to our office. A photo identification is required with all form submissions that request updates to name, address, phone numbers, and/or email addresses. Please review each form for any additional documentation that may be requested.
- In person – You may submit Records' forms to our office, located in Medlin Campus Center
Suite 3500 in Jamestown. We are open Monday-Thursday from 8 a.m. to 6 p.m. and Friday from 9 a.m. to 5 p.m. - eFax – You may fax us the form with the appropriate documentation to 336-458-2359.
